Sphincter augmentation with the GateKeeper procedure is a new option for the treatment of faecal incontinence.
The procedure was used in 3 women with faecal incontinence grade 3, relevant distress, and lack of conservative or surgical treatment options. On postoperative day 3, a magnetic resonance image (MRI) of the pelvis was obtained. A standard clinical examination was performed after 6 months and a standard telephone interview after 18 months.
The implantation was technically possible without complications. Postoperative symptoms were limited. In the MRI, the intersphincteric implants were hypointense and delineated with few artefacts. There was an improvement in the grade 3 incontinence. The leading symptom during follow-up was soiling.
The procedure is safe and effective. Further placebo-controlled studies are necessary to determine the value of the procedure in incontinence therapy.
